I need short-term services for a loved one who will be discharged from the hospital shortly. Can you help with this?

As many of our clients are referred to us by physicians, discharge planners, social workers and case managers with post-hospitalization needs, we are aware that the transition from hospital to home can be a difficult one for patients and their loved ones. Whether your loved one is going to a rehabilitation facility or directly to home, the presence of a dedicated care giver can help ensure that recovery runs smoothly. Home care can greatly facilitate the development and maintenance of the post-hospitalization routine. Caregivers can help patients with regular meal times, medication reminders, therapy schedules, follow-up medical appointments and more. In addition, the caregiver provides constant monitoring and support that is critical to patients with mobility or dexterity issues. Finally, the caregiver can allow the patient to focus on rest and recovery by taking on household and other daily responsibilities.

My loved one is a fall risk and needs constant supervision. Do you offer live-in care?

Live-In care is a cost-effective option for seniors who need an around the clock presence.For people who are looking for a comprehensive alternative to any type of facility in the Bay Area, or prefer the comfort and safety of round-the-clock monitoring and care, we offer 24/7 live-in care. Live-in care can be equated with in-home assisted living-all the support, monitoring and care of an assisted living facility but in the comfort of home.
